ABSTRACT

Objective:To study the correlation between systemic immune inflammation index (SII) and prognosis of patients with hilar cholangiocarcinoma after surgical treatment.Methods:The clinical data of 181 patients with hilar cholangiocarcinoma treated by surgery at the First Affiliated Hospital of Zhengzhou University from January 2012 to December 2016 were retrospectively analyzed. There were 119 men and 62 women, with an average age of 62.4 years. SII was calculated using preoperative routine blood tests. Receiver operating characteristic (ROC) curve was used to obtain the optimal cutoff value of SII. The Kaplan-Meier method was used to draw survival curves and survival rates were compared by log-rank test. The Cox proportional risk model was used to analyze single and multiple factors.Results:The SII area under the ROC curve in predicting postoperative survival was 0.749(95% CI: 0.641-0.858), the optimal threshold was 412.6. Using this threshold, patients were divided into the low SII group (SII≤412.6, n=80) and the high SII group (SII>412.6, n=101). The 1, 3, and 5-year cumulative survival rates of patients in the low SII group were 87.5%, 57.5%, and 26.3%, which were significantly better than those of the high SII group of 71.3%, 39.6%, and 9.9% respectively ( P<0.05). Multivariate analysis showed that SII>412.6 ( HR=2.887, 95% CI: 2.256-7.903, P<0.05) was an independent risk factor for overall survival of patients with hilar cholangiocarcinoma. Conclusion:Preoperative SII had predictive values for postoperative survival of patients with hilar cholangiocarcinoma, SII>412.6 was an independent risk factor for postoperative survival.

ABSTRACT

Objective To understand the current situation and predict the trends in number and composition of prenatal ultrasound screening staff in Beijing. Methods We analyzed the region,age,professional title and other characteristics of prenatal ultrasound screening personnel in Beijing during 2007-2015.We then built an ARIMA model basing on the current situation to predict the number and composition of the staff in 2016-2020. Results The number of prenatal ultrasound screening staff showed an upward trend in 2007-2020 and was predicted to reach 1269 in 2020.During this period,the educational achievement and professional title of the staff showed a downward trend,and the working years became shorter,mainly below 5 years.The proportion of resident doctors remained at 26.6%,and that of the staff receiving further education would reach 43.2% by the end of 2020. Conclusion The prediction under ARIMA model suggests that efforts should be made to strengthen the training of young doctors and provide them opportunities for further study.

ABSTRACT

OBJECTIVE@#To explore the role of follicular helper T cell (Tfh)/ follicular regulatory T cell (Tfr) imbalance in B-cell lymphoma (BCL).@*METHODS@#Sixteen BCL patients who were admitted to the Department of Hematology of The First People's Hospital of Yichang and 20 healthy people from December 2019 to November 2020 were enrolled and respectively divided into observation group and control group. The levels of Tfh and Tfr in peripheral blood were detected by flow cytometry. The changes of Tfh, Tfr, and Tfh/Tfr ratio were compared and the relationship between Tfh/Tfr ratio and efficacy, prognosis was analyzed.@*RESULTS@#Compared with the healthy controls, Tfh and Tfh/Tfr ratio in peripheral blood of the BCL patients increased (P<0.05, P<0.01), while levels of Tfr was decreased (P<0.01). After chemotherapy, Tfh and Tfh/Tfr ratio in peripheral blood of the BCL patients decreased significantly than before chemotherapy (P<0.01), but Tfr was no significant difference. Multivariate analysis showed that Tfh and Tfh/Tfr ratio were positively correlated with international prognostic index (IPI) score and Ann Arbor stage (r=0.626, 0.564, 0.573, 0.608, respectively), while Tfr negatively (r=-0.504, -0.542, respectively). According to the average value of Tfh/Tfr ratio at initial diagnosis, BCL patients were divided into Tfh/Tfr high ratio group and low ratio group. It was found that the complete remission (CR) rate, overall response rate (ORR), and survival time in the high ratio group were significantly lower than the low ratio group (P<0.01).@*CONCLUSION@#There is an imbalance of Tfh/Tfr ratio in peripheral blood of the BCL patients, and those with a high Tfh/Tfr ratio have lower CR, ORR and shorter survival time.

ABSTRACT

Objective:To analyze the clinical and imaging features, and treatment and curative effects of primary splenic angiosarcoma.Methods:A retrospective study was conducted on 6 patients with primary splenic angiosarcoma diagnosed histopathologically at the First Affiliated Hospital of Zhengzhou University from January 2012 to December 2019. The clinical characteristics, CT and ultrasound imaging findings, treatment methods and survival outcomes of these patients were analyzed.Results:There were 4 males and 2 females, with an average age of 52.6 years. These 6 patients with primary splenic angiosarcoma had no specific clinical manifestations, and the diagnoses were all confirmed histopathologically after surgery. CT examination of these 6 patients showed that the spleen volume was increased, with splenic space-occupying lesions. The lesions were single in 1 patient and multiple in 5 patients. Some lesions fused with each other. There were multiple intrahepatic metastases in 1 patient. Ultrasound in 3 patients showed enlarged solid hypoechoic spleens, and rich blood flow at the edges. Five patients underwent splenectomy with complete removal of tumors. For the patient with multiple lesions in the spleen and multiple space-occupying lesions in the liver who could not be treated radically, only splenectomy was done. This patient developed significantly more liver lesions 2 months after surgery, and survived for 3 months. Two patients who underwent postoperative adjuvant chemotherapy relapsed at 9 months and 13 months after surgery. They survived for 16 months and 19 months, respectively. A patient who received postoperative adjuvant chemotherapy survived for 27 months without any obvious signs of recurrence. A patient who underwent targeted therapy relapsed after 10 months, and survived for 14 months after surgery. The remaining patient who underwent simple surgery relapsed 4 months after surgery and died 6 months later.Conclusions:Primary splenic angiosarcoma is a highly malignant tumor. In this study, it lacked specific clinical manifestations and imaging features and its diagnosis required histopathological examination. Surgical resection is the main treatment for this disease. Whether surgery combined with postoperative adjuvant therapy is a more effective treatment than surgery alone for splenic angiosarcoma requires more studies in the future to determine.

ABSTRACT

Objective To learn the capability and functioning status of prenatal ultrasound screening institutions in Beijing and inform decision-making on the optimization of prenatal screening management. Method By using the data from the second and the third National Special Survey on Prenatal Screening and Prenatal Diagnostic Services in Beijing,we analyzed the distribution,human resources,workload,technology,and equipment of ultrasound screening institutions in Beijing as well as their changing trends. Results From 2013 to 2015,there were 100 prenatal ultrasound screening institutions,833 technicians,and 383 prenatal ultrasound screening instruments in Beijing,offering over 600 000 service sessions.In contrast,there were 93 institutions with a total service volume of 644 100 between 2010 and 2012.The general hospitals,tertiary hospitals,and urban areas(especially urban functional development areas and new urban development areas)were the main sources of the services.In particular,the maternal and child health care hospitals in Beijing provided the highest amount of services per unit of resources.The average prenatal ultrasound screening rate was about 98.05%.The service delivery trended to transfer from urban areas to remote county areas and from secondary institutions to tertiary institutions.Conclusion The operation and management of prenatal ultrasound screening institutions in Beijing are relatively standard.The ultrasound screening rate is relatively high in the second trimester,and the distribution of the services has constantly been improved since 2010.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the effect of Rhodiola on the expression of iNOS mRNA in severe acute pancreatitis (SAP) associated renal injury rats.</p><p><b>METHODS</b>A total of 72 healthy rats were randomly divided into the sham-operated group (S), the SAP associated renal injury group (M), and the Rhodiola-treated group (RHO), 24 in each group. Rats in S and M groups were peritoneally injected with 10 mL/kg saline 3h before modeling, while rats in the RHO group were peritoneally injected with 10 mL/kg Rhodiola Injection 3 h before modeling. The peripheral ligament of pancreas was bluntly dissociated in rats of M and RHO groups. The head of pancreas was occlused by nontraumatic blood vessel forceps 3 h later to establish the model. Eight rats were randomly selected from each group at 12, 24, and 36 h after modeling to detect levels of serum amylase, creatinine, and blood urea nitrogen. Serum levels of interleukin 1β (IL-1β) and interleukin 10 (IL-10) were detected by enzyme-linked immunosorbent assay (ELISA). Pathological changes of the left kidney were observed under light microscope. The expression of inducible nitric oxide synthase (iNOS) mRNA in the right kidney was detected with real time polymerase chain reaction (RT-PCR).</p><p><b>RESULTS</b>Compared with the S group, serum levels of amylase, creatinine (Cr), blood urea nitrogen (BUN), IL-1β, IL-10, and iNOS mRNA expression significantly increased in the M group (P < 0.01). The function of kidney and pancreas were obviously improved in the RHO group than in the M group. Levels of IL-1β and iNOS significantly decreased, but IL-10 levels significantly increased in the RHO group with statistical difference (P < 0.05).</p><p><b>CONCLUSION</b>Rhodiola had better protective effect on SAP associated renal injury, which might be achieved through inhibiting the expression of IL-1β, stimulating the expression of IL-10, down-regulating iNOS mRNA expression, reducing the generation of oxygen free radicals and NO damage to cells, and improving hypoxia tolerance capabilities of the kidney.</p>

ABSTRACT

To evaluate the efficacy of a variety of lacrimal duct obstruction diseases treated by Nd: YAG laser lacrimal duct forming surgery using Meta analysis. ●METHODS: Relevant database in 1995 - 2012 and other network resources were retrieved by computer and manual, using MetaAnalyst software on the Meta analysis. Analysis of indicators: disease prevalence between men and women, the prevalence of various lacrimal duct diseases and the effective rate of Nd: YAG laser lacrimal duct forming surgery to treat a variety of lacrimal duct obstruction diseases. ●RESULTS: According to the inclusion and exclusion criteria, 38 subjects were selected, the cumulative number of patients included 4638 cases, the men combined prevalence was 23%; the women combined prevalence was 77%; lacrimal punctum occlusion combined prevalence was 6. 0%, combined treatment efficiency was 88. 1%; lacrimal canaliculus obstruction combined prevalence of 17. 9%, combined efficiency was 85. 6%;lacrimal common canaliculus obstruction combined prevalence was 14. 0%, 92. 7% of the combined effective rate; nasolacrimal duct obstruction combined prevalence of was 43. 6%, 93. 3% of the combined effective rate;chronic dacryocystitis combined prevalence was 18. 4%, 76. 6% of the combined efficiency. All study has heterogeneity. Selection of random effects model combined effect size. Model was statistically significant (P ●CONCLUSlON: The prevalence of lacrimal duct obstruction diseases of women are higher than men, lacrimal canaliculus obstruction in the lacrimal duct obstruction is in the lowest prevalence, nasolacrimal duct obstruction is the main type of lacrimal duct obstruction disease, Nd: YAG laser lacrimal forming surgery has slightly efficacy different in various lacrimal duct obstruction diseases.

Based on the characteristics and core contents of military medical service commanding, the authors developed information equipment to improve the commanding ability of military medical service at emergency and wartime. To solve the complicated problems in the allocation of military medical service forces, a box for commanding military medical service was designed by integrating the geographic information system, decision support system and computer hardware technology, which provides portable equipment for military medical service commanders and allows them to rapidly have schemes for allocation of military medical service forces.
